NIRA Form 3 Download Preview

NIRA is the government body which takes on the Birth Certificate issuing and registration in Uganda. Known as the Uganda National Identification Registration Authority, it is the same body which is responsible for the National ID registration and issuance to qualifying citizens of Uganda, and which means that in order to get either a death or birth certificate or a national ID, you should provide them with all the required details.

Before, this was the work of the Uganda Registration Services Bureau (URSB) until when NIRA took over the above ever since the country introduced the National ID’s. If you are in need of a bithy certificate, it is mandatory that you obtain a short BC which you should present to NIRA offices together with the fully filled Form 3 you can obtain from their offices, and or download from online just like you can see the preview in this post.

This downloaded NIRA form 3 (regulation 8(1)) has many options to fill which include but not limited to the following below..

Like you see in the above screenshots, you need to fill that form with every requested detail and attach your short BC and submit the form to NIRA offices. Please note, the above form can’t be downloaded and used from here, it is for informational purposes only so that you can know what you need.

  1. Hello.I have turned eighteen this year (20th/dec/2018) I registered for the child mass enrollment of schools in 2017.but the ids have not come back and are for kids. Should I re.register for the adult national id that takes a maximum of 90days to come back. I have all the requirements.should I procede please because I need the id argently

  2. How long doesn’t it take to get a birth certificate and also changing mistake made in my id

    1. sennono stephern, the time it takes is determined by many factors including where you submit. One thing you should know is that upon submission, you are told when to pick your ready birth certificate. On the mistakes side, it all depends on the mistake made for example, you might be required to swear a court affidavit, advertise in the press and etc to correct name errors. Hope this helps.

  3. Lost a relative in 1994. He didn’t die in the hospital. Apparently the administrator general needs a death certificate. How do we go about this? Can we get a death certificate? What documents are needed?

    1. You need to download and fill NIRA Form 12 for notification of death…. You should have a short death certificate and a medical report indicating the cause of death as well.

    1. Thanks justice for the comment and inquiry. As per NIRA guidelines, Ugandan citizens should turn up for registration with any supporting documents. These may include Birth Certificate, Marriage Certificate, Academic certificates, Passport, LC letter etc. Birth certificates, Religious certificates, School leaving certificates, Child health card, Notification of birth, Letter of administrative officer, Parish/Sub county chief, Statutory Declarations, LC letter, Any other as the case maybe.

      You can go to your LC1 and get a letter. Have it stamped by the GISO and proceed to the Parish offices, Sub county office or KCCA division offices eg KCCA Rubaga, KCCA Makindye, KCCA Kawempe, KCCA Nakawa, etc according to where registration is taking place.

    I am writing on behalf of my sister who is disabled. We live in the UK since 1970, however my sister Hadjia Khan was born in Uganda at the Mulago Hospital on 11th October 1966.
    We have lost her long birth certificate, can you advise me on how I can obtain a replacement please?
